Welcome to our Electronic Medical Records (EMR) Patient Portal. Whether you are a new patient or existing patient of our practice, it is required that all patients complete their online registration through the new Patient Portal prior to your appointment. Rest assured the forms are SECURE and HIPAA compliant.

Patient Portal Access Instructions

Please follow the steps below to access your patient portal and complete any required forms:

Step 1: Access Your Secure Link

Use the secure link sent to you via email or text message from our office.

Step 2: Verify Your Identity

Enter your:

• Date of Birth
• ZIP Code

Select Continue to proceed to your account.

Step 3: Complete Required Items

Once logged in, you will be able to:

• View upcoming appointments
• Complete and submit required documents

Security Code Information

• In most cases, a security code is not required when accessing your account through the link provided by our office.
• If prompted for a security code, please refer to your original email, where a code may have been automatically generated.
• This typically applies only if you are accessing the portal outside of the direct link or linking accounts from another practice.

Important Notice

Our office does not have access to your unique security code. We recommend following the steps above before contacting us, as most access issues can be resolved using the original link provided.
